If you are currently enrolled in the transit benefit program and your original application on file is dated BEFORE October 19th 2016, you must submit a new application to satisfy annual recertification and continue to receive transit benefits.
You must submit the TSA form 205 application. You will check “RE-CERTIFICATION” at the top of the form. Please return to me at Suite 300 in a sealed envelope as soon as possible to avoid cancellation of your current benefits. Current enrollees , please check “re-certification”. If you have a new hire date AFTER October 19th 2016 re certification does NOT apply to you. If you would like to apply for transit benefits for the first time, please check NEW on the application.
